With over 20 years of experience working with complex chronic illness, we have developed an approach that goes far beyond simply managing your symptoms. Frankly, we wish it would be as simple as handing you a single prescription and sending you on your way. However, two decades of clinical experience have taught us otherwise.

CFS and Fibromyalgia are total-body illnesses, and total-body illnesses require a total-body solution. Our approach moves beyond simply naming your condition and suppressing its most disruptive symptoms. We look for the “why” behind your illness and address it at its source.

Two Poles of the Same Condition

Chronic Fatigue Syndrome is formally defined by fatigue lasting six months or more, while Fibromyalgia is characterized by body pain involving five or more areas persisting for three months or more.

Both conditions share a constellation of overlapping features: post-exertional malaise (feeling significantly worse after physical or mental effort), cognitive impairment, unrefreshing sleep, and orthostatic intolerance.

In our clinical view, CFS and Fibromyalgia are not two separate illnesses: they are two poles of the same underlying condition.

Some people present primarily with profound fatigue, others with widespread pain. Most experience both. The underlying mechanisms (neuroinflammation, immune dysregulation, mitochondrial dysfunction, and disrupted gut health) are remarkably similar regardless of which label a patient carries.

A diagnosis of CFS or Fibromyalgia should be the beginning of a root-cause investigation, not the conclusion of one.

Why Conventional Approaches Often Fall Short

The standard medical model struggles with CFS and Fibromyalgia because it was built for acute illness and isolated organ problems. If you have joint pain, you see a rheumatologist. If you have fatigue, you see an internist. If you have brain fog, you may be told the problem is psychiatric.

The result is that most patients spend years collecting diagnoses without ever receiving answers.

Compounding this is the persistent underestimation of missed underlying causes driving these conditions. In our own practice going back to 2004, we discovered that more than 30% of patients carrying a CFS or Fibromyalgia diagnosis tested positive for chronic Borrelia infection: Lyme disease. Many others had chronic activated viral infections, mold toxicity, or subtle hormonal and thyroid abnormalities that had been entirely overlooked.

These are not rare findings in a specialty practice like ours: they are common. The assembly-line approach of “one specialist, one organ, one prescription” will not solve a total-systems illness. To truly heal, we must adopt a systems-based framework that looks at the whole person.

The Essential First Step: What Must Be Ruled Out

Many people are diagnosed with CFS or Fibromyalgia without ever having had a careful root-cause evaluation. Before we can build a treatment plan, we must ensure we are not missing a primary, treatable driver of your illness. The most important conditions to rule out include:

  • Chronic Tick-Borne Infections: Lyme disease (Borrelia) and co-infections, including Bartonella and Babesia, can produce a clinical picture identical to CFS or Fibromyalgia. Standard Lyme testing through conventional labs misses a significant number of cases. When indicated, we use sensitive immunoblot testing through IGenex and microscopic evaluation through TLabs.

  • Chronic Activated Viral Infections: Epstein-Barr Virus (EBV), Human Herpesvirus 6 (HHV-6), and Cytomegalovirus (CMV) can reactivate in the setting of immune dysregulation and drive ongoing fatigue, pain, and cognitive symptoms that look exactly like CFS or Fibromyalgia.

  • Intestinal Yeast Overgrowth: Too many yeast in the intestines produce “leaky gut,” leading to brain microglia inflammation, fatigue, and malaise. Stool testing often misses intestinal yeast. We often make a clinical diagnosis of yeast considering symptoms and your history of medications and stress that lead to this problem.

  • Mold Toxicity: Mold toxin illness, caused by living or working in a water-damaged building, produces a clinical presentation virtually indistinguishable from CFS, Fibromyalgia, or Lyme disease. If your symptoms began or worsened after a move or a water damage event, mold must be seriously considered. For many patients, addressing mold toxicity is the single most important intervention we make.

Only once we have evaluated for these root causes can we build a truly targeted recovery plan.

Functional Medicine provides the “why”: a deep root-cause analysis using a detailed timeline of your health history to identify every possible influence, from genetic predispositions and infectious triggers, to environmental toxins and hormonal disruptions.

The Roadmap to Recovery

Recovery from CFS and Fibromyalgia is like peeling the layers of an onion: each layer addressed in sequence creates the conditions for deeper healing. Our protocol targets the underlying problems driving your symptoms, rather than simply suppressing them.

1. Sleep, Diet, & Inflammation Control

Recovery cannot happen in a body that is constantly in crisis mode. Chronic immune activation triggers cytokines that disrupt sleep architecture, worsen pain, and suppress immune function.

We strengthen restorative sleep using sleep hygiene with targeted supplements or sleep regulating devices like Appollo Neuro. We pair this with a plant-forward, Mediterranean-style diet and targeted gut microbiome testing through Jona Health to optimize butyrate and short-chain fatty acid production. Liposomal curcumin and other antioxidants help turn down the inflammatory signaling that keeps so many patients stuck.

2. Addressing Root Causes & System Failures

Many patients remain ill because system failures beyond the primary diagnosis are blocking recovery. We look carefully for the following:

  • Microglial Activation: Chronic activation of the microglia (the brain’s own immune cells) due to intestinal dysbiosis or systemic infections amplifies pain signals and perpetuates immune dysregulation throughout the body. We address this with Low Dose Naltrexone (LDN): one of the most effective and well-tolerated tools we have for both pain reduction and immune regulation in CFS and Fibromyalgia.

  • Hormonal & Thyroid Dysfunction: Many patients have subtle thyroid dysfunction that standard TSH testing misses. We look specifically for peripheral thyroid resistance by measuring elevated reverse T3, and may use pharmaceutical T3 to restore cellular thyroid action. For perimenopausal and menopausal women, HRT can meaningfully improve immune function, cognitive health, and pain.

  • Mitochondrial Dysfunction: Mitochondrial failure is one of the most consistently documented biological abnormalities in CFS and Fibromyalgia. We use a targeted repair protocol including phospholipids, CoQ10, pyrroloquinoline quinone (PQQ), essential micronutrients, and creatine monohydrate to restore your cells’ energy production capacity.

  • Adrenal Support: HPA axis dysregulation is common in both conditions. We use adaptogens like Ashwagandha to support the stress axis without suppressing natural cortisol rhythm.

3. Pain Management: Targeted & Evidence-Based

We treat pain as a signal pointing to addressable biology, not a condition to be managed indefinitely with opioids. Our toolkit includes LDN as a first-line option, with FDA-approved Duloxetine (Cymbalta) and Pregabalin (Lyrica) available when clinically appropriate. Liposomal curcumin addresses the cytokine-driven inflammatory component of pain in parallel.

Importantly, based on the 2021 UK NICE Guidelines, we do not use graded exercise therapy: research clearly shows that pushing through fatigue worsens post-exertional malaise and delays recovery. Movement is reintroduced carefully, only when the underlying biology supports it.

4. Brain Training: Retraining the Limbic System

In many patients with CFS and Fibromyalgia, the limbic system becomes stuck in an “on” position: chronically signaling the body to remain in a state of illness and threat response. This is a measurable neurological phenomenon, not a psychiatric problem.

We work with validated programs including The Gupta Method, Primal Trust, and the Dynamic Neural Retraining System (DNRS). University studies of the Gupta Method show symptom improvements of 50% or more in a meaningful proportion of participants, with approximately one-third experiencing this level of benefit from brain retraining alone, making it an essential component of a comprehensive recovery plan.

5. Treating Underlying Infections When Present

When our evaluation identifies chronic tick-borne infections or activated viruses as contributors to your illness, we treat them directly using both herbal and prescription antimicrobials or targeted antivirals guided by laboratory findings. We believe in treating the whole picture, not just the label on the chart.

We think outside the box. Low Dose Naltrexone, mitochondrial repair protocols, microbiome optimization guided by advanced stool testing, limbic retraining, and careful hormonal and thyroid assessment are not part of most conventional CFS or Fibromyalgia care. They are part of ours though, because they work. We also use compounded therapies when they offer a clinical advantage, and we are always willing to revisit the plan as your condition evolves.
